IMPORTANT INFORMATION

We were just notified that UofL is requiring every student athlete to fill out and sign a Youth Protection Program Form for every event that a youth athlete participates in on their campus this season. This is new, and other sports at Manual have been required to fill these out this year as well.

We will need every swimmer, ESPECIALLY THOSE WHO ARE PARTICIPATING IN THE FIRST TWO MEETS AT UOFL to fill out the form attached.

Please sign and email these forms back to us as soon as possible so we can be sure to get them in for the upcoming meet this weekend. We will try to have extra copies at practice tomorrow and at the meet if you do not have access to a printer. Parents must sign the forms if the athlete is under 18 years of age.
